2019 - 2020 team Membership requirements
LA DIve CLub members will need to fulfill the following requirements prior to their first practice to be part of the program here at UCLA.....
1) USA Diving Registration: You must register with USA Diving to participate. You can do this at https://webpoint.usadiving.org/wp/Memberships/Join.wp. This is an annual fee, and runs September 1 - December 31 before having to register for the 2019 year starting January 1-December 31. Competitive Team athletes will need to be registered Competitive Athletes ($75). Grey Team, Blue Team and Bruin Bouncers and Lesson program athletes may register as Athlete ($20). If you do move up within the program, you can always pay the difference to upgrade.
***For USA Diving we are considered Los Angeles Dive Club and are part of the Southern Pacific Diving Association***
2) Amateur Athletic Union (AAU) Diving Registration: You must register with AAU to participate. This is an annual fee, and runs September 1 - August 31. All athletes should register as Youth Athlete extended coverage ($14). You can do this at http://www.diveaau.org/Membership/SignUp.aspx and go to the "Athlete Register Now" link on the front page. You will need to enter our AAU Club Code for the 2019-2020 year which is WY7YWF.
***For AAU Registration we are considered Los Angeles Dive Club***
3) UCLA Waiver: This form must be completed annually. Please click on the UCLA Waiver - or use the link https://pdf.ac/6Xfeee . Everyone must turn in this form prior to ANY level of participation.
4) Emergency Contact Form: Please fill out an emergency contact form by going here, so the coaches have all the necessary information, in the event it is needed. When filling out the emergency contact forms please include your USA Diving number and your AAU Diving number in the space provided on the form. This information is needed before the first day of practice. You can also PRINT the emergency contact form, and bring it to your first practice.
5) Parent and Diver Responsibilities Form: Parents, please read through the expectations we have established for you and read through those we have established for your athletes with them. We are asking this year that everyone sign these forms and return them to us so that we all have a clear understanding of the process we believe is critical to your child's success. That form may be electronically submitted here.
6) Parent and Diver Concussion Information Sheet: Parents, please read through the Information sheet provided on concussions. That form may be electronically submitted here.
7) Team Membership Fee: The Membership fee is primarily used to purchase new equipment (i.e. new diving boards, mats, belts, etc.) we use throughout the year, facility upgrades (i.e. new cameras for the TIVO’s, dryland upgrades, etc.), and cover all administrative costs to run our program.
8) Session Coaching Fees: The Session Coaching fees (Lesson/Competitive Team) are due on the 10th day of each session. If they are late, there is a $25.00 late fee. If for any reason a diver needs to take a break or take a session off, we need 24 days advanced notice, if we do not we will have to charge for the session. Once a diver starts a session, we do not offer refunds or Pro-rate the session for team members.
